Featured Report: EHR use up among family doctors, but varies by area

The number of family physicians who have adopted electronic health records has more than doubled since 2005, though wide geographic variations exist, according to a report in the Annals of Family Medicine.

Using census survey data from the American Board of Family Medicine maintenance of certification exam and the National Ambulatory Medical Care Survey, researchers predicted that the adoption rate could pass 80% by the end of the year.

In the NAMCS, adoption among family physicians grew to 66.4% in 2011 from 24.8% in 2005. Among physicians undergoing the ABFM's maintenance of certification, adoption increased to 67.8% in 2011 from 28% in 2005.

The study notes “how federal efforts to increase adoption of EHRs have accelerated in recent years.” It adds that the federal government's “triple aim” goals to improve population health and healthcare delivery while lowering costs “will require data sharing and exchange that transects all aspects of healthcare delivery and depend in part on widespread adoption of EHRs, particularly by office-based physicians

Feds offer loan repayments for new docs

While the argument over whether there is a national physician shortage has not been settled, there should soon be at least about 87 more primary-care doctors working in underserved areas where the lack of a healthcare workforce is not subject to debate. The new doctors—spread out among 29 states, Washington and Puerto Rico—will split more than $10 million in medical school loan repayments issued by the National Health Services Corps. Up to $120,000 is being allocated to fourth-year primary-care medical students in exchange for a commitment to practice full-time for three years or part-time for six years in designated areas upon completion of their residency training.

“The issue of how to match patients with their medical records needs to be solved as we accelerate information exchange on regional and national levels,” according to the AHA. “The inability to match patients across silos raises safety concerns about mismatches--incorrectly matching patients, or missing a match that should have been made. In addition, without a single, national approach to patient matching, hospitals and health systems are forced to expend significant resources on expensive, proprietary solutions to develop master patient indexes that apply only to that particular hospital or health system’s patients.”

Busy docs, inadequate IT systems hinder shared decision-making

Implementing shared decision-making isn't as easy as it sounds, according to a study by the RAND Corp. and Informed Medical Decisions Foundation. In an article published in Health Affairs, the authors follow a demonstration of shared decision-making at eight primary care sites that was funded and coordinated by the foundation. From the project's first 18 months, the authors said the biggest barriers were time-pressed physicians, lack of training about shared decision-making and inadequate IT systems.
