Medication errors can be a big problem for healthcare providers. Patients being prescribed medication that conflicts with drugs they may be taking, but have not disclosed, is one of the biggest sources of these errors. The problem is this information may not come to the attention of healthcare professionals until it’s too late, according to a recent report produced by the ECRI Institute for the Pennsylvania Patient Safety Authority. Almost 900 medication errors were reported in a seven year time frame from July 1, 2004, through January 2011, involving patients taking their own medications while in healthcare facilities. One or more controlled substances were involved in more than 40 percent of events reported to the PPSA with more than 25 percent involving high-alert medications. Oxycodone topped the list of patients personal medications implicated in medication errors — it was involved in 77 reported cases or 8.8 percent. Although the top five medications on the list are controlled substances, insulin accounted for 34 cases. Metropolol, used to treat cardiovascular disease, was listed in 25 cases.

Planned Parenthood settles with Illinois on Medicaid payments

Planned Parenthood of Illinois has agreed to pay the state $367,000 to settle a dispute over alleged overbilling of the Medicaid program by the not-for-profit's medical director. Dr. Caroline Hoke, an obstetrician-gynecologist and the organization's medical director since 2007, had been under threat of termination from the state Medicaid program since 2010, when the inspector general of the Department of Healthcare and Family Services sought to recover allegedly improper payments.

Microsoft-supported health appstore launches in UK

A Cambridge, UK-based startup called Cambridge Healthcare just launched a health-specific app store for patients in the UK. The store is actually part of a personal health platform, a PHR that is available to patients in the country’s National Health Service (NHS) system, for free. The offering is called: How are you? In recent months Cambridge Healthcare has inked deals with Microsoft to integrate its offering with HealthVault — and add a couple of Microsoft executives, including the company’s Chief Architect of Healthcare Solutions, Sean Nolan, to its board.
